The Aboriginal Health Team can provide personalized care to
Aboriginal patients to manage and follow their care plan, understand their
medicines, conduct home visits and assist with health assessments.
What
is a chronic disease?
A chronic
disease is a health condition that is persistent or otherwise long-lasting in
its effect. The term chronic is usually applied when the course of the disease
last more than three months.
